Privacy and receipt
The form records your name, email, contract reference, optional contract date, declaration, receipt number and submission time. This is used to process the withdrawal and provide evidence; it does not automatically decide eligibility or cancel records in the external calendar. The acknowledgement is returned immediately as a text file you can save. Contact the provider if you need assistance or a refund update.
